Cory Monteith, a cast member in the television series "Glee," poses before a screening and Q&A for the show, at the Academy of Television Arts and Sciences in Los Angeles, on May 1, 2012.

According to the final coroner’s report, Cory Monteith had been shooting heroin and drinking champagne before his death this summer. Responders discovered a “spoon with drug residue and a used hypodermic needle” in the Glee star’s room, along with two empty champagne bottles. Official cause of death was listed as “mixed drug toxicity, involving intravenous heroin use combined with the ingestion of alcohol,” and suggested that since the actor had stopped using after rehab in April, his tolerance for the substances were probably lower.
