Earlier this year more than 40 mostly rural counties risked lacking a provider when sign-ups begin in November, though they have since found coverage. Insurers have been pulling out of the exchanges after struggling with heavy losses and failing to attract young, healthy people to the program. The future of the program is also uncertain, since President Trump has repeatedly threatened to end government payments that help cover cost-sharing reductions for customers with modest incomes.
