At least six people—including American military "leaders"—could face a disciplinary review over the burning of Qurans at a U.S. base in Afghanistan, according to a source close to the joint Afghan-American investigation. New, conflicting accounts of the incident emerged Saturday. Both American and Afghan officials investigating the Quran burning say destruction could have been avoided were it not for a chain of mishaps, poor judgments, and ignored procedures. President Obama and other U.S. officials have apologized profusely and said the Feb. 20 Quran burning was an accident, but their apologies have not quelled anger, though six days of deadly riots across the nation have lost some momentum.
