Because 26 states voted against expanding Medicaid.

Though the Affordable Care Act aimed to get health insurance for more poor Americans by requiring states to expand Medicaid, 26 states refused to do so and created a coverage gap instead. As a result, up to 7 million Americans are now ineligible for Medicaid. All 26 of the states that opted to create a coverage gap had Republican-controlled legislatures or Republican governors. Coincidence?
